What Is a Medicare Chronic Disease Management (CDM) Plan?
The Chronic Disease Management (CDM) Program—formerly known as the Enhanced Primary Care (EPC) program—is a Medicare-funded initiative that helps people with long-term health conditions access coordinated care. If you have a chronic medical condition lasting longer than six months, such as lower back pain, arthritis, cardiovascular conditions, diabetes, or post-surgical rehabilitation needs, you may be eligible for a CDM plan.
A CDM plan is created by your GP to help manage your condition more effectively. It outlines your health needs and goals and sets up a structured treatment plan. As part of this, your GP may prepare Team Care Arrangements, allowing you to receive Medicare-subsidised support from allied health professionals, such as Exercise Physiologists.
What Is Bulk-Billing and Gap-Free Exercise Physiology?
Accredited Exercise Physiologist is part of the allied health team eligible to provide services under the Medicare Chronic Disease Management program.

✔ $0 per session (for up to 5 Medicare-subsidised sessions per year)
Do I need a referral for Medicare bulk billing?
Yes — you do need a referral for Medicare Chronic Disease Management (CDM) services. To access Medicare-subsidised Exercise Physiology sessions, your GP must complete the EPC/CDM referral letter/form.
Only your General Practitioner can create the CDM plan and issue this referral, as specialists and other health providers are not able to.
